Your privacy is our priority. Although often a full chart is requested by insurers or other treatment providers, we do not recommend releasing your full file to them. For instance, if you are requesting time off of work for FMLA or accommodations where you live, these other entities do not need to know your entire life story to approve your requests. The same is true for requests related to legal matters and continued care with other providers. These requests can be approved without you waiving your right to privacy. We recommend releasing something like an attendance report or treatment summary rather than your full file, although you have a right to release what you want.
Records can only be requested by the client on record or their legal guardian. Records will only be released once the identity of the client or their guardian is confirmed. A member of the clinical team will reach out once the request is received and processed. Should we need any additional information or signed forms, we will collect them at that time.
